Legal Services Benefit User Agreement:  Please fill out this form and give it to your attorney on your first visit. The user agreement authorizes the plan administrator to use your legal services benefit when paying your attorney. It serves as a contract between you and the Legal Trust Plan. Your user agreement also notifies the attorney to bill for services according to the APEA-AFT Legal Trust Plan rates.

Case Description Form: Print this form and give it to your attorney. The attorney will complete the form and send it to the Legal Trust office along with the first invoice and a copy of your user agreement. The case description form indicates the specific matter for which you are seeking legal services and is required to determine coverage for your claim. If you have a question as to whether your matter will be covered by the APEA-AFT Legal Trust, please contact the plan administrator before obtaining legal services.

Plan Attorney Agreement Form: Plan attorneys agree to restrict their rates for all our participants. They are reimbursed in full up to the maximum yearly legal services benefit. Plan attorneys are listed on our web site.

Non-Plan Attorney Agreement Form: Attorneys who do not necessarily restrict their hourly rate but are willing to work with some of our participants are reimbursed at a lower rate, according to the terms of the Non-Plan Attorney Agreement. The client is responsible for any balance remaining after the Legal Trust payment.


If the attorney you choose does not have an agreement on file with our office, ask the attorney to sign one of the attorney agreement forms in order to be paid by the Legal Trust Plan.


Please present your attorney with the necessary forms to ensure proper billing. These can be printed from the website or obtained from the Plan office. Each covered bargaining unit is listed on the User Agreement form. If you are employed in one of these bargaining units, you have the APEA-AFT Legal Services Benefit.
